TO ENSURE YOUR FIRST VISIT WITH US IS A SEAMLESS EXPERIENCE, PLEASE BRING THE FOLLOWING TO YOUR INITIAL APPOINTMENT:
- Radiographs (X-rays) if applicable
- A list of your current medications and dosages
- Your dental insurance information, if applicable
It is important and helpful to our dental team that you communicate to them if you have a medical condition that may impact your dental procedure.
Conditions that may affect your procedure include diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.
You should also inform us if you take any medications that may interfere with those we prescribe. These can include heart medications, aspirin, anticoagulant therapy, and many others. If you require medication prior to a dental hygiene cleaning, please inform our team well in advance.
All patients under the age of 18 must be accompanied by a parent or guardian at their initial examination and consultation appointment. Thank you and please contact our office if you have any questions at all.
Payment Options
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
Dental Insurance
As a courtesy to our patients, our practice accepts assignment of benefits from most dental insurance companies. We do require a credit card to be left on file for this option. Our administration staff will prepare all the necessary forms for your dental benefits. However, we remind you that your specific policy is an agreement between you and your insurance company. Due to the privacy act we cannot be held responsible to know the details (maximums, frequency) of your coverage. Please keep in mind that you are responsible for your patient portion should your insurance benefits result in less coverage than anticipated. Our staff will gladly submit a pre-treatment estimate to your insurance company so that you are aware of what your benefits will cover.
The fees charged for services rendered to those who are insured are the usual and customary fees charged to all our patients for similar services. Your policy may base its allowances on a fixed fee schedule, which may or may not coincide with our usual fees. You should be aware that different insurance companies vary greatly in the types of coverage available. Also, some companies take care of claims promptly while others delay payment for several months.
